Mind My Health is a free, easy-to use online advance care planning tool that provides universal access for individuals to “own their health, and plan for care in advance.” Using Mind My Health’s innovative, intuitive platform, users store their legally-recognized advance care plan (Health Care Power of Attorney, Living Will, or Advance Directive) in on our secure, cloud-based registry. Users also may use Mind My Health to share their documents with loved ones and their care team, which makes their decisions known directly at the point of care, whenever needed.
Who
Mind My Health is a project of The Carolinas Center for Hospice and End of Life Care (TCC), a not-for-profit association serving hospices and healthcare systems in North Carolina and South Carolina. Mind My Health was created with generous support from The Duke Endowment and also received funds to provide organ donor information from the North Carolina License to Give Trust Fund Commission, a division of the NC Department of Administration. Mind My Health is designed for use by any adult who has completed, or is ready to complete, an advance directive.
What
Mind My Health is an online platform that is available on all devices (desktop, tablet, phone).
Where
Mind My Health’s website is available anytime, anywhere. Users can upload documents from whatever state they call home, but Mind My Health only provides blank documents for users in the Carolinas at this time.
Why
Advance Care Plans allow individuals to communicate their wishes for their healthcare if they are no longer able to speak for themselves and are associated with a wide range of benefits for patients, families, and health teams. Too often, however, individuals store their advance care plans in inaccessible locations such as a locked box, leaving their families unable to locate them in the very moments in which they are needed. Mind My Health’s secure online storage allows users to safely store their documents and share them with loved ones so they can be confident that their documents will be accessible when they are needed.
When
Mind My Health launched in the spring of 2019, after 2 years of focused work on the design and technology that would create an online site that is completely secure, easy to use, and easy to share with family members and healthcare providers.
Other Important Details
In the Hospice world, we often see how important faith is for many people as they participate in Advance Care Planning. Faith colors the way many people engage in such conversations and often times faith leaders are looked to for help and advice in these matters. As such, we are excited to have a chance to partner further with faith leaders (both ordained and lay) in hopes of increasing conversations around end of life care planning in a way that meets people where they are in a culturally competent manner. We are happy to provide training resources and opportunities for leaders who wish to bring this work to their communities and help patients and their families ensure that the care provided aligns with their wishes.
